Coverage
The Plan
pays the
first
$150.00/per
year for
a
routine
physical
exam
(physician
portion).
$250.00/per

Diagnostic
X-Ray

Laboratory
work.
Once
these
maximums
are
reached,

balance
is
applied
to the
Major
Medical
Portion
of the
Plan.

calendar

deductible,
per
person,

once the
deductible
has been
met the
balances

covered
services
are paid
at 80%
of the
Usual
and
Customary
charges.
Medical
coverage

provides

Emergency
Room
visits

emergency
reasons

Optical
Benefits

months

each
member
and
eligible
dependent.
Dental
Services
are
covered
at 80%
of the
Usual,

charge
with a
yearly
maximum
of
$1,500.00.
The Fund
also has
its own
preferred
provider
arrangement.
If
services
are
performed
by a
participating
dentist,
benefits
are
payable
at 100%
up to
the
$1500.00
yearly
maximum.
Certain
services
are
covered
under
the
Surgical
portion
of the
Plan and
are
payable
at 90%.
There
is NO
Orthodontic
Coverage.
You are
also
entitled
to
Prescription
Drug
Coverage
at $5.00
for
generic,
$10.00
for
named
brand
for a
thirty
day
supply,
up to a
maximum
of
$7,000.00
per
year.
Once the
$7,000.00
is used
the Plan
will pay
80% of
the
allowed
amount.
There
is a
mail
order
prescription
drug
benefit
available
for
maintenance
drugs,
copayments
are the
same
however,
you will
receive
a 90 day
supply
for the
same
copayment
as a one
month
supply
at the
pharmacy.

Vesting
Service

Plan
year, in
which
you earn

hours of
service

Vested
Pension
A
regular
pension
is
available
as early
as age
62,
provided
a member
has five
vesting
credits,
(five
years of
service)
(as
required
by
Federal
Law).
Benefits

current

monthly.
For the
five
year
period
based on
the
current
employer
contribution

hour,
your

Minimum
of Ten
Years of
Service:
One

hours
worked

credit.
You may
actually
earn
more
than 1
year of
credit

example,

hours
would
give a
member

service
credits

Regular
Pension
Upon
reaching
aged 62

the

rate,
your
employer

Disability
Pension
Upon
total
and
permanent
disability,
awarded
by the
Social
Security
Administration,
a member
could
retire,
at any
age
prior to
age 62,
with ten
years of
service,
provided
he/she
worked
in
covered
employment
or made
self
contributions
for a
total of
at least
2000
hours in
the
period
consisting
of the
Plan
Year in
which
you
became
totally
and
permanently
disabled,
and the
two
previous
plan
years.
Death
Benefit
If you
should
die
prior to
retirement,
your
beneficiary
is
entitled
to
receive
a refund
of the
contributions
made in
your
behalf
prior to
becoming
vested
or once
vested,
50% of
the
benefit
you
would
have
received
had you
retired
the day
before
you died
and
elected
the
Joint &
Survivor
Form of
payment.
Benefit
is
payable
when you
would
have
been 62
or age
55 if
you have
10 or
more
service
credits.
Spouse
can
elect a
lump sum
payment
instead
of a
monthly
award.
If you
are not
married,
your
beneficiary
will
receive
a lump
sum
payment
of the
contributions
made in
your
behalf.
Post
Retirement
Death
Benefits
If no
Joint &
Survivor
Annuity
is
chosen,
the
difference,
if any,
between
contributions
made in
your
behalf
and the
amount
of
benefits
paid
while
retired
is
payable
as a
lump
sum.
If a
Joint &
Survivor
Annuity
is
chosen,
your
spouse
gets a
percentage
of your
benefit
usually
50% but
can be
75% or
100%.
If you
retire
directly
from
active
employment,
you can
elect a
Joint &
Survivor
Annuity

that if

spouse
dies
before

your

Medical
Coverage
is
offered
to
retirees
who have
maintained
10 out
of 12
years of
eligibility
prior to
retirement

60 – 62
the cost

62-Medicare

Surviving
Spouses
and
Dependents
of
Deceased
Employees
or
Retirees
If
you are
eligible
under
the
Medical
Plan,
whether
active
or
retired,
upon
your
death,
your
spouse
and
eligible
dependent
children
will be
extended
coverage
at no
cost for
one full
year
(unless
spouse
remarries
within
that one
year).
If you
are an
eligible
member
(active
or
retired),
and you
had
reached
age 50
at the
time of
your
death,
your
spouse
can
continue
to
receive
benefits
after
the one
year
period,
the
current
rates
for
widows
are
$66.00
monthly
age 65
and
over,
$118.00
monthly
under
age 65.
If you
have not
obtained
age 50
at the
time of
your
death,
after
the one
year

your
spouse
will be
offered
COBRA
Continuation
Coverage.
Retirement
Savings
Plan
If you
receive
1.5% of
your
contribution
to the
company
401K
Plan
which
averages
to about
$18.00/weekly
based on
40
hours.
Under
the
Union
Retirement
Savings
Plan
(which
is a
self
directed
Annuity
type
Plan),
the
contribution
is
currently
$1.35
per hour
which
average
to
$54.00
weekly,
all
employer
contributions.
There is
immediate
vesting
for all
contributions.
Currently
the
contributions
are
placed
in a
guaranteed
fund
until
the
participant
elects
to
change
investment
options.
The
change
of
investment
options
can be
done via
internet,
phone or
mail.
You can
collect
the
balance
of your
account
when you
retire
or
separate
from
employment
for a
period
of
twelve
months.
At the
time of
your
death,
your
beneficiary
will be
entitled
to
receive
the
balance
in your
account.
Existing
401K
funds
can be
rolled
over
into the
Retirement
Savings
Plan.
